Angels of Mercy Private Homecare Services provides in-home private duty care and care management services for seniors throughout the Middle Georgia area. Our CNA (certified nursing aides), caregiver, and companion services allow our clients to age in the place where they feel most comfortable and secure - at home. Combine our Home Care Services, supervised by RNs, with our Geriatric Care Management component, and we provide the most comprehensive assistance for the Middle Georgia areas seniors.

Our Geriatric care manager works privately with older adults and their families to create a plan of care that meets the needs of the older adult. They will meet with you to help you understand your loved one’s needs and learn what resources and options are available to meet those needs.

What is a Geriatric care manager?
A Geriatric Care Manager (GCM) is a professional who develops and implements a plan of care to assist elders and their families with all aspects of long-term care. American culture has become transient. Families are often separated by distance and many children find themselves juggling demanding careers, families, and the responsibility of caring for an aging parent. It would be ideal to live in a society where our parents and our elderly could be cared for easily, where extended families were in close proximity, and where both family members were not working full time. The reality is that for most of us this is not the case. There are many older people who live alone, far from children and other relatives. Even for those who live near children, the fact remains that all family members may be working and may not be in a position to do all that is required. When older people begin to have trouble coping with their daily lives, they often cannot turn to family members for help, and often, even more importantly, they do not wish to burden those they love with their care. Under these circumstance a Geriatric Care Manager can be very useful.

 Advocacy in Long Term Facilities

Our Geriatric care managers can serve as an advocate for an older adult in a long term care facility on behalf of a long distance family member. Our Geriatric care managers are aware of the state regulations and structure of these facilities. Therefore, they know what to look for when visiting a facility, what questions to ask, and who to direct questions to in order to obtain the information needed to make the best decision. Our Geriatric care managers should be thought of as true advocates for the older adult in the facility.

· Aging is a fact of life. As your parents age their care needs change. This is an issue many families try to delay for as long as possible. The reality of the matter is that as loves ones ages their health condition declines. Some declines are gradual and some are immediate. Some questions you should ask yourself:

  • How prepare are you for the change?
  • What are the wishes of your senior loved one?
  • Does your loved have adequate funds available to cover the expenses of medical and non-medical care if needed?
  • Does your loved one have adequate health insurance coverage?

Medicare does not cover 24-hour-a-day care at home, meal delivery, homemaker services like shopping and laundry. So what does medicare really cover?

  • Does your loved one need a Medicare supplemental policy?
  • All of these are very pertinent questions that should be addressed. Often these issues are addressed too late.

Rise-N-Shine / Fluff-N-Tuck
Assistance is provided in which an aide goes in to assist with the morning care . This includes personal care assistance with bathing, dressing and grooming, prepare breakfast and/or lunch. This also may provide light housekeeping.

Rise N Shine Fluff N Tuck helps the elderly person to start their day. Then in the evening an aide will go back to the home to assist with the evening meal and to assist with preparation for bed. This service is available for the elderly who may need help getting in and out of the bed.

TLC Recovery Assistance
TLC Recovery is services available for a person who is recuperates from an acute illness, an accident or surgery. This services is a short term service that is available until an individual.

Caregiver’s Relief / Respite
Caring for a family member in the home can be an exhausting task.
Caregiver’s burnout is very common for family members who are directly in charge in the day to day care of disabled or elderly loved ones. Caregiver’s relief allows the family members to take a break from care giving tasks. Family members are able to get the much need rest and relaxation with the assurance that your loved one is in the care of Angels

New Mom’s Care
It is an exciting time to be a new mom. We know how stressful it can be too!

Whether bed bound by pregnancy or adjusting to the new life and family, this can be both exciting and exhausting. Don’t let the little things get in your way of adjustment.
At Angel of Mercy Private Homecare Services, we’re able to design a package of services to meet your needs as a new mom both before and after giving birth. With services to assist you in your daily chores, you’re given the opportunity to relax before giving birth and spend time after giving birth to bond with your new baby.

New Mom’s Care Includes Assistance With:

  • Light Housekeeping
  • Meal Planning and Preparation
  • Grocery Shopping
  • Laundry
  • Linen Changes
  • Errands
  • Bathing (hands on or standby)

Our Geriatric care managers can make referrals to other professionals in the community for needed supportive services. Some examples of this can include the following: referrals to psychiatrists, psychologists, psychotherapists, physicians and other medical professionals, elder care lawyers, realtors, financial planners, trust officers, etc.

In some cases the older adult may need on-going monitoring in order to maintain the highest level of independence in their home. The older adult may not have family members in town or family members that are bound by other obligations that are unable to assist with necessary daily programs. In this case, our Geriatric care managers can step in and provide regular support and supervision to the older adult. Routine reports can then be given, either in written format through a letter and/or e-mail or through verbal communication via phone, to concerned family members or other professionals regarding the on-going status of the older adult.

Long Term Placement in Continuing Care Retirement Communities, Assisted Living Facilities or Nursing Homes

If, for some reason, it is no longer safe and appropriate for the older adult to remain at their home, our Geriatric care managers can navigate the maze of long term placement steps and state required paperwork. Our Geriatric care managers can also assist older adults and/or their family members in coping with the emotions of transition from home to facility.
